An elderly individual has just been discharged from hospital following a stroke and is now recuperating at home. What services are available to assist him or her in recovery?
A
We recommend that a physiotherapist (referred by a registered medical practitioner) conduct a Rehabilitation Needs Assessment, followed by the development of a targeted rehabilitation plan. The therapist and carer will determine the appropriate follow-up, whether by a therapist, health worker, or physiotherapy assistant.
We can also arrange for an occupational therapist to perform a Home Modification Requirement Assessment , identifying safety risks related to mobility and suggesting improvements.
If the individual experiences swallowing difficulties, a speech therapist can conduct a Swallowing Ability Assessment and develop a targeted training or treatment plan.
Additionally, stroke survivors may require special care with daily activities such as eating, personal hygiene, and follow-up appointments. They can utilise TotalCare's Home Care Services to book medical escorts, personal care assistants, or health workers.

What is the difference between personal care assistants and health workers?
A
All personal care assistants and health workers appointed by TotalCare have undergone professional training and possess relevant experience. Their competencies are assessed and supervised by a TotalCare registered nurse to ensure they provide professional and attentive care to the elderly, including assistance with bathing, toileting, dressing, changing, feeding, monitoring vital signs, and medication reminders. Health workers receive additional training in medical procedures, such as basic wound care, nasogastric tube feeding, and assisting healthcare professionals with rehabilitation and meal planning.
