Haven of Hope Day Care Cum Rehabilitation Centre for Elderly (Hong Kong East)
Overview
Our team of nurses, registered social workers, occupational therapists/physiotherapists, and health service workers provides one-stop case management for each elderly individual. In our day care centre, we offer comprehensive care and training. We also provide home care options, including personal care, rehabilitation and cognitive training, short-term accommodation, and point-to-point escort services. Our services help elderly individuals maintain their quality of life, and our social workers offer counseling to alleviate caregiver stress.
Targets
- Individuals holding Community Care Service Vouchers (CCSV) issued by the Social Welfare Department.
- Elderly individuals without any infectious diseases.
- Applicants must undergo a professional assessment to confirm eligibility.
- Non-CCSV holders can access the centre's self-financing services.
Services
Basic Nursing
Regular health checks, including vital signs (blood pressure, temperature, etc.), blood sugar testing when needed, medication guidance, and wound care.
Meals
Balanced and nutritious breakfast, lunch, and snacks.
Personal Care
Daily living assistance and personal care services, including feeding and toileting training.
Rehabilitation Exercises
Physiotherapists and occupational therapists develop appropriate exercises and training for elderly individuals, guiding them in using the centre’s facilities.
Home Care Services
Each elderly individual collaborates with a case manager to establish a "Personal Care Plan," providing services based on the chosen service package.
