Haven of Hope Lok Wah Day Care Centre for the Elderly
Overview
Our team of nurses, registered social workers, occupational therapists/physiotherapists, and health service workers provides one-stop case management for each elderly individual. In our day care centre, we offer comprehensive care and training. We also provide home care options, including personal care, rehabilitation and cognitive training, short-term accommodation, and point-to-point escort services. Our services help elderly individuals maintain their quality of life, and our social workers offer counseling to alleviate caregiver stress.
Targets
- Individuals holding Community Care Service Vouchers (CCSV) issued by the Social Welfare Department.
- Elderly individuals without any infectious diseases.
- Applicants must undergo a professional assessment to confirm eligibility.
- Non-CCSV holders can access the centre's self-financing services.
Services
Our centre provides both centre-based and home-based services, including rehabilitation exercises, nursing services, personal care, short-term accommodation, and day respite care.
Each elderly individual works with a case manager to discuss and establish a "Personal Care Plan," with services provided based on the selected service package.
