Professional Assessment

Our team of professionals conducts assessments on specific needs for caring, rehabilitation, cognitive ability, swallowing ability, and home safety. This helps us understand the elderly individual's physical and mental condition. Based on the findings, we develop targeted caring plans, rehabilitation and cognitive training, and support measures as professional recommendations.

Nursing and Caring Needs Assessment

Conducted by registered or enrolled nurses, this assessment evaluates the health status, abilities, medical history, medication usage, dietary habits, preferences, and home care support of elderly individuals. The aim is to provide proper care guidelines for frontline carers and family members. 

We strongly recommend this detailed assessment before arranging home care services to ensure appropriate support for the elderly.

Rehabilitation Needs Assessment

Provided by physiotherapists (referred by registered medical practitioners), this assessment evaluates the physical and functional conditions of elderly individuals. Personalised rehabilitation plans are developed to promote optimal recovery in a familiar environment, with regular follow-ups to adjust plans based on changing needs. 

This service is suitable for post-operative recovery, stroke rehabilitation, maintenance of bodily functions, and injury rehabilitation.

Cognitive Ability Assessment

Conducted by occupational therapists (referred by registered medical practitioners), this assessment uses tools like the Montreal Cognitive Assessment (MoCA) to evaluate cognitive abilities and daily living challenges. Areas assessed include attention, memory, language skills, and executive function, leading to tailored cognitive training plans. Regular follow-ups ensure adjustments are made to enhance cognitive function and daily skills.

Swallowing Ability Assessment

Provided by speech therapists (referred by registered medical practitioners), this service assesses swallowing function to create targeted training or treatment plans for safe and effective eating and drinking. Using professional tools, therapists evaluate muscle coordination, swallowing reflexes, and safety during meals, delivering a detailed report and personalised training plan based on individual needs.

Home Modification Requirement Assessment

Conducted by occupational therapists (referred by registered medical practitioners), this service assesses the home environment to identify safety hazards and obstacles related to mobility and daily habits. 

Therapists provide targeted recommendations, such as installing handrails or optimising lighting. 

If necessary, they can issue formal letters to property management or government departments to request home modifications, assisting elderly individuals in obtaining needed support.
